What Is Semaglutide, and How Does It Work for Weight Management?

If you have been researching medical weight management in Canada, you have almost certainly come across the name semaglutide — often under its brand names, Ozempic and Wegovy. It has become one of the most discussed treatments in the country, and also one of the most misunderstood.

This guide explains what semaglutide is, how it works in the body, what the clinical evidence shows, who it is authorized for in Canada, and what to consider before starting treatment. The goal is to give you a clear, accurate picture so you can have a more informed conversation with a licensed healthcare provider.

This article is educational and is not a substitute for medical advice. Semaglutide is a prescription medication, and eligibility is determined through a medical consultation.

What is semaglutide?

Semaglutide is a medication known as a GLP-1 receptor agonist. GLP-1 stands for glucagon-like peptide-1, a hormone your body produces naturally in the gut after you eat. Semaglutide is a synthetic version of that hormone, designed to last far longer in the body than the natural version — which is why it is taken just once a week.

In Canada, semaglutide is sold under different brand names for different purposes:

  • Ozempic is authorized by Health Canada for the management of type 2 diabetes in adults.

  • Wegovy is authorized by Health Canada for chronic weight management.

  • Rybelsus is an oral tablet form authorized for type 2 diabetes.

Although Ozempic and Wegovy contain the same active molecule, they are approved at different doses and for different indications. Using Ozempic for weight management in a person without type 2 diabetes is considered an off-label use that Health Canada has not authorized for that purpose. This is an important distinction, and one a licensed provider will discuss with you.

In 2026, Health Canada also began authorizing generic versions of semaglutide, which are pharmaceutically equivalent to the brand-name products and have expanded lower-cost access for Canadians.

How does semaglutide work?

Semaglutide works by mimicking the GLP-1 hormone your body already makes. That single mechanism produces several effects that support weight management:

  • It signals fullness. GLP-1 receptor agonists act on areas of the brain that regulate appetite, helping you feel satisfied sooner and stay satisfied longer.

  • It slows digestion. By slowing how quickly your stomach empties, semaglutide helps reduce the frequency and intensity of hunger.

  • It supports blood sugar regulation. GLP-1 improves the body's insulin response after eating and reduces the release of glucagon, a hormone that raises blood sugar. This is why the same molecule is used to manage type 2 diabetes.

The practical result for many people is a meaningful reduction in appetite and food intake. Importantly, this is a physiological effect, not a matter of willpower — the medication changes the hormonal signals that drive hunger.

What does the clinical evidence show?

The most influential study on semaglutide for weight management is the STEP 1 trial, published in the New England Journal of Medicine in 2021.

In that trial, adults with overweight or obesity who received once-weekly semaglutide 2.4 mg alongside lifestyle changes lost an average of 14.9% of their body weight over 68 weeks, compared with 2.4% in the placebo group. About 86% of participants taking semaglutide lost at least 5% of their body weight.

It is worth emphasizing two things the trial makes clear. First, semaglutide was studied as an adjunct to a reduced-calorie diet and increased physical activity — not as a replacement for them. Second, results vary between individuals, and the averages above describe a study population, not a guarantee for any one person.

Longer-term data from the STEP 5 trial found that weight loss was sustained over two years of continued treatment, reinforcing that this is a long-term approach rather than a short-term fix.

Who is semaglutide for in Canada?

Health Canada authorized semaglutide (Wegovy) for chronic weight management in adults who have:

  • a body mass index (BMI) of 30 kg/m² or greater (classified as obesity), or

  • a BMI of 27 kg/m² or greater (classified as overweight) with at least one weight-related health condition, such as high blood pressure, type 2 diabetes, high cholesterol, or obstructive sleep apnea.

The Canadian Adult Obesity Clinical Practice Guideline recognizes semaglutide as one of several medications that can produce clinically significant weight loss as part of a broader obesity-management plan.

Eligibility is not something you determine on your own. A licensed healthcare provider will review your medical history, current medications, and health goals to decide whether semaglutide is appropriate for you. Semaglutide is not suitable for everyone, including people with certain personal or family medical histories, and this is exactly what a medical consultation is designed to assess.

What are the side effects?

Like any medication, semaglutide can cause side effects. The most common are gastrointestinal — including nausea, vomiting, diarrhea, constipation, and abdominal pain. In clinical trials, these effects were most often mild to moderate and tended to ease over time.

To reduce the likelihood and severity of these effects, semaglutide is started at a low dose and increased gradually over several weeks — a process called dose escalation. This is one of the reasons ongoing clinical support matters throughout treatment.

Less common but more serious risks exist as well, which is why semaglutide should only ever be taken under the supervision of a licensed healthcare provider. Always review the product monograph and discuss your full medical history with your provider, and do not stop or change your treatment without speaking to them first.

What happens if you stop taking it?

Semaglutide manages weight while you are taking it; it is not a permanent cure for obesity, which is a chronic condition. Research on the STEP 1 trial extension found that many people regained a substantial portion of lost weight in the year after stopping treatment. This is not a personal failing — it reflects the underlying biology of obesity as a chronic disease, and it is why treatment is generally approached as a long-term commitment paired with sustainable lifestyle changes.

The bottom line

Semaglutide is a well-studied, Health Canada–authorized option for chronic weight management that works by mimicking a natural gut hormone to reduce appetite and support fullness. The clinical evidence is strong, but it works best as one part of a broader plan that includes realistic, repeatable lifestyle changes — and only when prescribed and monitored by a licensed healthcare provider.

If you are wondering whether semaglutide could be right for you, the next step is a medical consultation. At Kinro, that consultation, your prescription if you are eligible, and ongoing clinical support are all coordinated through our own pharmacy, so your care stays connected from start to finish.

This article is for general educational purposes and does not constitute medical advice, diagnosis, or treatment, and does not list every factor affecting eligibility. Semaglutide is a prescription medication authorized by Health Canada; it is not suitable for everyone, and eligibility is determined through a medical consultation with a licensed healthcare provider. Always consult your provider and review the product monograph before starting any treatment. Individual results vary.

  • Semaglutide is a GLP-1 receptor agonist that mimics a natural gut hormone to reduce appetite and support fullness — it's the active ingredient in Ozempic and Wegovy.

  • In the STEP 1 trial, adults with overweight or obesity lost an average of 14.9% of their body weight over 68 weeks alongside lifestyle changes.

  • It's a prescription medication authorized by Health Canada for chronic weight management; eligibility is determined through a medical consultation.

Is semaglutide the same as Ozempic and Wegovy?
Ozempic and Wegovy both contain semaglutide as their active ingredient, but they're authorized for different uses and doses in Canada. Ozempic is authorized for type 2 diabetes; Wegovy is authorized for chronic weight management. Using Ozempic for weight loss in someone without diabetes is an off-label use Health Canada hasn't approved for that purpose.

How much weight can you lose on semaglutide?
In the STEP 1 clinical trial, adults taking semaglutide 2.4 mg alongside lifestyle changes lost an average of 14.9% of their body weight over 68 weeks, and about 86% lost at least 5%. Individual results vary, and it works best combined with diet and physical activity.

Who is eligible for semaglutide in Canada?
Health Canada authorized semaglutide (Wegovy) for adults with a BMI of 30 or higher, or a BMI of 27 or higher with at least one weight-related health condition. Eligibility is determined by a licensed healthcare provider through a medical consultation.

What are the most common side effects?
The most common side effects are gastrointestinal — nausea, vomiting, diarrhea, and constipation. In trials these were usually mild to moderate and eased over time. Starting at a low dose and increasing gradually helps reduce them.

What happens if you stop taking semaglutide?
Research shows many people regain a significant portion of lost weight after stopping, because obesity is a chronic condition. Treatment is generally approached as a long-term commitment alongside sustainable lifestyle changes.
