GLP-1s and Sexual Health: What to Know

As GLP-1 medications like semaglutide have become widely used for weight management, people are asking good questions about how they affect other parts of health — including sexual health, fertility, and contraception. Some of these effects are positive; some are important precautions everyone on a GLP-1 should understand.

This guide covers what's currently known about GLP-1s and libido, an important and sometimes surprising effect on fertility, and the contraception and pregnancy considerations that matter — especially for women and people who can become pregnant.

Libido and sexual function

The evidence here is still developing and individual responses vary, so this isn't a guaranteed effect. The fertility effect people don't expect

This is the part everyone on a GLP-1 who can become pregnant should know about, because it catches people off guard: weight loss can restore fertility.

Excess weight can suppress ovulation and reduce fertility, particularly in conditions like PCOS. When GLP-1 treatment leads to significant weight loss and improved metabolic health, it can restore ovulation — sometimes in people who previously had difficulty conceiving and assumed their pregnancy risk was low.

The result is a phenomenon that's become common enough to earn a nickname: so-called "GLP-1 babies" — unexpected pregnancies in people taking these medications, sometimes even while using contraception. If you can become pregnant, the practical takeaway is important: don't assume low fertility just because conceiving was difficult before. That can change on a GLP-1.

Contraception: why it matters more on a GLP-1

There are two separate reasons contraception deserves attention while on these medications:

1. Restored fertility (above). If weight loss restores ovulation, reliable contraception becomes more important than it may have been before — especially if pregnancy isn't the goal.

2. Possible effects on oral contraceptive absorption. This is where the specific medication matters:

  • Semaglutide (Ozempic, Wegovy) is not currently expected to significantly reduce the absorption of oral contraceptives, based on available research. However, that research is limited.

  • Tirzepatide (Mounjaro/Zepbound) is different — its labeling notes it can reduce oral contraceptive absorption, and a backup method or a switch to non-oral contraception is generally advised, particularly when starting or increasing the dose.

There's also a practical wrinkle that applies to any GLP-1: these medications can cause nausea, vomiting, and diarrhea, especially early on. If you vomit or have significant diarrhea shortly after taking an oral contraceptive pill, it may not be fully absorbed — so following the missed-pill guidance or using backup contraception is sensible.

Non-oral contraceptive methods that bypass the digestive system — such as IUDs, implants, injections, patches, or rings — aren't subject to these absorption concerns, which is one reason a provider may suggest them.

Regulatory bodies in some countries have specifically warned that people on GLP-1/GIP medications should use effective contraception, after reports of unintended pregnancies. This is exactly the kind of thing to confirm with your own provider based on your specific medication.

Pregnancy: GLP-1s are not for use while pregnant

GLP-1 medications are not recommended during pregnancy. Product labeling generally advises stopping semaglutide well before a planned pregnancy — often cited as around two months before trying to conceive — because the medication clears the body slowly and safety data in pregnancy is limited.

If you're planning a pregnancy, or if you become pregnant while taking a GLP-1, this is a conversation to have with your provider promptly. If pregnancy occurs unexpectedly on a GLP-1, contact your provider — the general guidance is to stop the medication and be assessed.
